Instructions:
- Personalize your medal by typing your name and the date in the space provided on the PDF.
- Print the template on regular paper or cardstock and cut out the pieces. Make all folds along dashed lines
- Using glue or double-sided tape, secure all tabs to their corresponding pieces in alphabetical order.
- Punch a hole where indicated and thread a string or ribbon through the hole.
- Use the string or ribbon to wear the medal around your neck.
